Please complete the Intake form specific to your gender and the 2 different symptoms questionnaires at least 48 hours before your consultation. The Terms of Engagement and GDPR consent forms must be read and if in agreement signed before we can start working together. Please read the Heidi AI Medical Scribe information leaflet and indicate at our consultation if you wish to opt-out. This technology improves the accuracy of my notes and the quality of our interaction as I can focus entirely on our conversation without taking notes. There is an additional document titled “Required Blood Test Parameters" which outlines the comprehensive standard biochemistry I would recommend in the majority of cases for establishing the causes of your problems & ensuring no serious conditions are missed. We may discuss additional advanced functional medicine testing at our consultation. Please consider the options covered prior to our consultation & acquire previous results where possible.
